A heraldic-style bookplate in brown ink on ivory paper. The central shield is striped and bears a six-pointed star, and axe, and a crescent moon. The shield is topped with a crown bearing another axe, and above this is a flat-brimmed hat with ropes hanging from either side. Each rope makes a figure eight loop, and has a tassel on the end, which has two tassels hanging from it, and three tassels hanging from the second row. Beneath is the text "Ex libris Mons. Dr. Ant. Okolo-Kulak, Antist. Urb. Prael. Capit. Mohilovien."
